Qualifications

• Current RN license in TX and good standing.
• Experience in home health or related healthcare setting preferred.
• Strong clinical and assessment skills.
• Excellent communication and interpersonal abilities.
• Ability to work independently and manage time effectively.

Responsibilities

• Conduct initial and ongoing comprehensive assessments of patient needs and documentation.
• Determine admission and level of care for home health services.
• Develop and revise the plan of care to ensure quality and continuity.
• Educate patient and family on disease processes and care needs; provide training to staff as needed.
• Coordinate with the IDG to determine service scope and frequency based on acuity.
• Deliver nursing procedures and infection control practices.
• Provide clinical directions to aides and LPN/LVN to ensure quality care.
• Prepare progress notes and ensure documentation supports care goals.
• Inform physicians of changes and monitor patient responses to interventions.
