Qualifications

Current RN license in good standing with the state board.
Experience with Homecare Homebase EMR is a plus.
Experience in home health or related healthcare setting preferred.
Strong clinical assessment skills are required.
Excellent verbal and written communication abilities.
Ability to work independently and manage time effectively.

Responsibilities

Conduct initial and ongoing assessments of patients and their needs.
Determine eligibility and appropriate levels of care for home health services.
Develop and revise care plans to ensure quality and continuity of care.
Educate patients and families on disease processes, self-care, and end-of-life considerations.
Assess acuity to tailor service frequency and scope for each patient.
Provide clinical directions to aides and LPN/LVN to ensure quality care.
Document progress notes and outcomes to demonstrate goal achievement.
Communicate patient changes to physicians and the care team.
