Job summary

Parkhouse Nursing and Rehab Center in Royersford, PA is seeking a compassionate RN Registered Nurse to join our team. You will provide patient-centered care in a warm, welcoming environment and support residents with their healthcare needs.

You will collaborate with an interdisciplinary team, document in the electronic medical record, and contribute to developing care plans. Ongoing education and adherence to best practices help promote positive outcomes for residents and their families.

Qualifications

• Direct patient care for nursing residents including assessments and care planning.
• Wound care, ostomy care, and medication management as part of daily duties.
• Collaboration with interdisciplinary team to coordinate care.
• Documentation of patient information in EMR and tracking progress.

Responsibilities

• Provide skilled nursing care to residents, including wound care, ostomy care, and medication management
• Conduct initial and ongoing assessments to identify care needs and develop individualized care plans
• Implement and monitor care plans, making adjustments as necessary to ensure optimal outcomes
• Communicate effectively with residents, families, and interdisciplinary team members to ensure coordinated care
• Participate in quality improvement initiatives to drive better patient outcomes and resident satisfaction
