Qualifications

• Associates Nursing or Diploma (Nurse) Nursing or Bachelors Nursing-Preferred
• Experience with patient-centered care and interdisciplinary teamwork is valued.

Responsibilities

• Performs the Nursing Process (assessment, diagnosis, outcomes, planning, implementation and evaluation) with evidence-based practice
• Provides relationship-based patient-centered care that respects cultural diversity
• Partners with patient and significant others using bedside shift report and interdisciplinary rounds
• Coordinates delivery and documentation of safe, quality patient care with diverse team members
• Promotes performance improvement and shared governance within the care team
• Plans for care across the care continuum and supports protocol development with care managers
