Job summary

Reliant Management seeks a compassionate Registered Nurse (RN) to join a 52-bed Skilled Nursing Facility in California. This full-time, direct-hire role focuses on delivering high-quality resident care within a collaborative, resident-centered environment.

The RN will assess residents, develop care plans, coordinate with the interdisciplinary team, and ensure regulatory-compliant documentation and outcomes. Leadership abilities and supervision of LVNs/CNAs are valued.

Qualifications

• California RN license required.
• CPR/BLS certification required.
• Experience in Skilled Nursing Facility, Rehabilitation, or Long-Term Care preferred.
• Experience supervising LVNs and CNAs.
• Strong clinical assessment and critical thinking.
• Excellent communication, documentation, and problem-solving abilities.
• Ability to work independently while contributing to a team.
• Commitment to compassionate, resident-focused care.

Responsibilities

• Assess residents and develop individualized care plans.
• Coordinate care with the interdisciplinary team.
• Monitor resident conditions and communicate changes to physicians and leadership.
• Document assessments and treatments per regulatory standards.
• Administer medications, IV therapies, and treatments as prescribed.
• Collaborate with physicians, therapists, social services, dietary staff, and others.
• Supervise LVNs and CNAs.
• Maintain a safe, respectful, and therapeutic environment.
• Perform other duties as assigned.
