Mattapan Health & Rehabilitation Center is seeking a compassionate Registered Nurse (RN) to join our resident-focused care team. You will provide direct nursing care, administer medications, and develop individualized care plans to support residents’ health, safety, and quality of life.

Collaboration with physicians and interdisciplinary teams is essential, along with accurate documentation, HIPAA compliance, and commitment to professional excellence.

Qualifications

• Graduate of an accredited nursing program.
• Current Registered Nurse license in good standing with the applicable state licensing board.
• Long-term care experience preferred.
• Current required certifications and credentials.
• Strong clinical assessment and critical thinking skills.
• Compassionate approach to resident care.
• Ability to communicate effectively with residents, families, and healthcare professionals.

Responsibilities

• Provide direct nursing care to residents using the nursing process of assessment, planning, intervention, implementation, and evaluation.
• Complete comprehensive assessments and monitor changes in resident condition.
• Develop, implement, and evaluate individualized care plans in collaboration with residents, families, and interdisciplinary teams.
• Administer medications and treatments safely and accurately according to physician orders and facility policies.
• Monitor and document vital signs, clinical observations, treatments, and resident responses.
• Assist residents with activities of daily living while promoting independence, dignity, and self-esteem.
• Recognize and respond to changes in physical, emotional, cognitive, and psychosocial needs.
• Support residents with chronic conditions, rehabilitation needs, and complex healthcare challenges.
• Collaborate with physicians, nursing leadership, rehabilitation teams, and other healthcare professionals to support resident outcomes.
• Educate residents and families regarding care needs, treatment plans, and health management.
• Promote resident participation in their healthcare decisions whenever possible.
• Maintain accurate, timely, and complete clinical documentation.
