Job summary

HealthFirst Family Care Center Inc. in the Lakes Region seeks a compassionate Registered Nurse to join a collaborative primary care and behavioral health team. The RN will support health maintenance, disease management, and patient education.

Responsibilities include obtaining vitals, triaging calls, administering medications, assisting with immunizations, and contributing to performance improvement initiatives. NH RN license and BLS are required.

Qualifications

• Graduate of a Nursing Education Program.
• Associate's or Bachelor's Degree in Nursing.
• Current unrestricted RN license in NH.
• Current BLS certification at time of hire.
• One year of nursing experience preferred.

Responsibilities

• Provide nursing care at patient appointments.
• Take vital signs and patient history.
• Educate patients about acute and chronic conditions.
• Triaging calls and scheduling visits as needed.
• Administer medications and treatments per provider orders.
• Assist with venipuncture and specimen collection as ordered.
• Maintain and document patient data in the EMR.
• Support care coordination and population health initiatives.
