Qualifications

• Currently licensed RN in state of practice required.
• Must hold and maintain CPR certification.

Responsibilities

• Deliver high-quality nursing care focused on resident outcomes using the nursing process.
• Identify needs and input for discharge planning and follow-up care.
• Collaborate with residents, families and interdisciplinary team to develop and implement care plans.
• Ensure care plans are implemented consistently, efficiently and safely.
• Share information with physicians and other health care professionals to adapt plans of care.
