Job summary

Qualifications

• Current RN license in state of practice required.
• CPR certification required; must obtain CPR within 90 days of hire if not current.
• License in good standing with no abuse/neglect findings.

Responsibilities

• Deliver quality nursing care focused on outcomes using the nursing process.
• Identify discharge planning needs and provide follow-up throughout stay.
• Develop individualized care plans with the resident and interdisciplinary care team.
• Implement plan of care consistently, effectively and cost efficiently with resident-centered outcomes.
• Collaborate with physicians and other health professionals by sharing information relevant to changing plan of care.
• Maintain optimum resident care and comfort with up-to-date nursing practices.
