Interim HealthCare Inc. in Rock Hill, SC is seeking a full-time Registered Nurse (RN) to provide hospice and home health care in clients' homes.

The RN will assess, plan, and implement nursing care per individualized care plans, working autonomously yet collaborating with an interdisciplinary team. The role emphasizes compassionate patient education, meticulous documentation, adherence to state and federal regulations, and maintaining a safe, professional practice in home settings.

Competenze

• Valid and current RN license in North Carolina.
• Minimum of one year nursing experience, preferably in home health or similar setting.
• Excellent clinical skills and the ability to work independently.
• Strong communication and documentation skills.
• Ability to prioritize and manage multiple tasks; reliable transportation and valid driver’s license.

Mansioni

• Provide skilled nursing care to clients in their homes according to individual care plans.
• Perform comprehensive assessments, develop and implement care plans, and monitor progress.
• Administer medications and treatments as prescribed by the physician.
• Educate clients and families on disease processes, medications, and self-care techniques.
• Coordinate care with interdisciplinary team members, including therapists, social workers, and home health aides.
