Job summary

Empower Home Health - a LiveWell Partners Network Agency is seeking a compassionate Registered Nurse (RN) Case Manager to oversee care plans for patients in their homes. You will develop a Plan of Care, coordinate with physicians and the care team, and work to improve patient independence and satisfaction.

You will join a supportive clinical team, participate in on-call rotations, and maintain thorough documentation and communication.

Qualifications

• Minimum of 2 years of nursing experience with a preference for 2+ years in an acute or a post-acute care setting.
• Active State Registered Nurse licensure is required.
• Proof of current CPR certification and valid Driver’s license.

Responsibilities

• Assumes responsibility for assigned patient’s care from admission through discharge by effectively executing patient’s Plan of Care.
• Collaborates with the patient, family/caregiver(s), ordering physicians, other care team members, and community resources.
• Submit accurate, complete, timely, meaningful, and required documentation, including progress and clinical notes.
• Participate in case conferences with other clinicians by reviewing services, clinical documentation, and discussing pertinent patient care issues and goals.
• Participate in on-call rotation as reasonably assigned.
