Job summary

The Hillcrest of North Dallas is seeking a PRN Transportation Coordinator to coordinate and execute resident transportation to medical appointments under licensed nursing supervision.

Responsibilities include organizing timely rides, maintaining safety, and communicating with clinical staff. CNA certification is preferred but not required, with BLS/CPR certification encouraged to ensure resident well-being during transit.

Qualifications

• Valid driver's license for the state where applying.
• Nurse Aide Training/Competency Evaluation preferred but not required.
• Current BLS/CPR certification (as required by state).

Responsibilities

• Organizes transportation of residents to appointments based on needs.
• Transports residents to inpatient/outpatient programs and facilities.
• Develops driving schedule around appointments and transports after hospital discharge.
• Reports changes in resident condition to clinical supervisor.
• Transports medical records for signatures to medical offices.
• Maintains safety and vehicle cleanliness; logs mileage and maintenance.
• Assists clinical staff with lifting/transferring as ordered.
• Establishes communications with outside facilities to coordinate services.
• Reports scheduling changes to DON and staff; follows up as needed.
• Provides hydration to residents daily per needs.
• Protects resident property.
