Job summary

Greentree SNF Healthcare LLC is seeking a dedicated Certified Nursing Assistant to provide routine daily nursing care per resident care plans. You will identify residents, assist with daily activities, and ensure accurate documentation while maintaining a warm, safe environment.

Responsibilities include reporting changes to supervision, assisting with transfers, and upholding confidentiality and facility policies. A minimum 10th grade education, CNA license, and CPR preferred are expected.

Qualifications

• Must possess at least a 10th grade education.
• Licensed Certified Nursing Assistant (CNA) per state laws; CPR preferred.
• Ability to read, analyze, and interpret general business materials and procedures.
• Ability to write reports and procedures.
• Ability to present information clearly to managers and staff.
• Basic math skills for calculating measurements and doses.

Responsibilities

• Identify residents before providing care.
• Document entries accurately on flow sheets and charts.
• Report changes in resident condition to Nurse Supervisor.
• Maintain cleanliness and order in resident rooms and common areas.
• Assist with daily activities and personal care.
• Follow safety and confidentiality policies for resident information.
• Assist with transfers, mobility, and turning as trained.
• Participate in nursing shift reports and quality improvement.
