Qualifications

• Must be CBRF Certified in Wisconsin.
• High School Diploma or equivalent required.
• Proven ability to work independently while being a proactive team player.
• Basic computer skills for electronic charting and communication.
• Previous long-term care or assisted living experience preferred.

Responsibilities

• Provide hands-on assistance with Activities of Daily Living (ADLs).
• Monitor and track changes in resident physical condition and behavior.
• Support residents with medication reminders or administration.
• Encourage and assist residents with scheduled activities and community events.
• Maintain a safe, hygienic, and organized living space for residents.
• Ensure all care services are recorded accurately in the documentation system.
• Adhere to all healthcare guidelines for regulatory excellence.
