Qualifications
Must be CBRF Certified in Wisconsin.
High School Diploma or equivalent required.
Proven ability to work independently while being a proactive team player.
Basic computer skills for electronic charting and communication.
Previous long-term care or assisted living experience preferred.
Responsibilities
Provide hands-on assistance with Activities of Daily Living (ADLs).
Monitor and track changes in resident physical condition and behavior.
Support residents with medication reminders or administration.
Encourage and assist residents with scheduled activities and community events.
Maintain a safe, hygienic, and organized living space for residents.
Ensure all care services are recorded accurately in the documentation system.
Adhere to all healthcare guidelines for regulatory excellence.
