Responsibilities

• Provide skilled nursing services under physician or midlevel provider direction.
• Collect patient health data and analyze to determine diagnoses and outcomes.
• Develop plan of care with interventions to attain outcomes.
• Implement interventions and evaluate patient progress.
• Work collaboratively with the healthcare team.
• Perform professional nursing work with safety and infection-control focus.
• Educate patients and families about disease, care and treatment.
