Job summary

Qualifications

• Associate degree required and licensed/certified to practice in the state.
• Current CPR certification required.
• Two years of acute care, long-term care, or home health experience preferred.

Responsibilities

• Provide direct therapeutic care and teach rehabilitation techniques to patients and caregivers.
• Perform PTA-supervised treatments within scope, regulations, and policies.
• Update and revise patient plans of care based on assessments and goals.
• Document all skilled services timely and accurately for compliance.
• Educate patients and families to support ongoing progress and safety.
• Collaborate with the physical therapy team and other care providers.
