Job summary

Responsibilities

• Provide treatment to residents as directed by Physical Therapist.
• Assist in implementing individualized therapy plans.
• Complete timely, accurate documentation per regulatory and facility requirements.
• Maintain safe treatment environment and infection control.
• Communicate progress with interdisciplinary team and families.
• Participate in care plan meetings and rehab conferences.
• Educate residents and families on goals, exercises, safety, and discharge planning.
• Assist with training restorative aides and rehab programs.
