Job summary

The Community Mental Health Center (CMHC) seeks a Qualified Psychiatric Rehabilitation Practitioner to support individuals with chronic mental illness through the Intensive Psychiatric Rehabilitation Service (IPRS). Under supervision, you apply rehabilitation philosophy to help clients reach personal goals.

You will conduct triage, readiness assessments, create crisis plans, deliver IPR phases, and collaborate with staff; maintain records and ensure compliance.

Qualifications

• Bachelor’s degree in a human services field with 60 hours IPRS training.
• Criminal background check clearance and appropriate licensing.
• At least one year of service delivery to CMHC populations.

Responsibilities

• Ensure informed consent and complete all IPR releases per policy.
• Represent the program at community events and recruit clients.
• Conduct triage interviews to determine IPR eligibility.
• Complete readiness assessments, service plans, and periodic updates.
• Document social history within 30 days of admission.
• Develop individualized crisis plans with each client.
• Deliver all phases of IPR: Triage to Goal Keeping.
• Meet with clients individually and in cohorts for 4–10 hours weekly.
• Complete prior authorizations for IPR funding.
• Transport clients safely when needed.
