Job summary

Qualifications

• One year of Home Health Aide experience in home, community based or LTC setting.
• Compassionate, kind and patient focused
• Exceptional customer service and interpersonal skills

Responsibilities

• Planning. Assist in planning and implementing outings for Participants; provide daily documentation of patient care.
• Patient Care. Perform patient care such as feeding, grooming and transferring as determined by their care plan; prepare and distribute meals and snacks; assist with recreation/leisure activities; report any changes in patient status to Senior Program Assistant or Activity Coordinator in a timely manner
• Customer Service. Greet participants upon arrival; remove coats and guide to program area; prepare participants for departure
