Job summary

Qualifications
- Graduate of an approved Licensed Vocational program and licensed in the state of practice.
- Minimum 2 years nursing experience in skilled care preferred.
- Current BLS/CPR certification required.
Responsibilities
- Deliver nursing care per standards, regulations and policy.
- Admit, transfer and discharge residents with care documentation.
- Mentor staff, coordinate shift assignments and communicate with physicians.
- Administer medications and execute physician orders per policy.
- Ensure PCC and MDS processes reflect care plans and compliance.
- Promote resident/family satisfaction and high-quality outcomes.
- Investigate incidents and concerns, document and address findings.
- Assist in surveys, audits, and cross-department collaboration.
