Responsibilities

• Conduct comprehensive physical assessments focusing on mobility, balance, and gait.
• Develop individualized treatment plans with clear goals for mobility and pain reduction.
• Administer physical therapy interventions, including exercises, manual techniques, and modalities.
• Educate residents, families, and care staff on safe mobility techniques.
• Document evaluations, plans, daily notes and progress in EMRs (Net Health/PCC).
• Collaborate with the rehab team to ensure comprehensive care.
