Job summary

Height Street Skilled Center in Bakersfield, CA is seeking a licensed Physical Therapist (PT) to join our rehabilitation team at a long-term care facility. You will assess patients, develop individualized treatment plans, and collaborate with physicians and staff to improve mobility and independence.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current and valid Physical Therapist license in the State of California.
• CPR certification (current and valid).
• Experience in a skilled nursing or long-term care setting preferred, but not required.
• Strong communication and documentation skills.
• Ability to motivate and encourage patients in a compassionate, respectful manner.

Responsibilities

• Evaluate and assess patients to develop individualized treatment plans
• Provide physical therapy services to patients recovering from injury, illness, or surgery
• Collaborate with physicians, nursing staff, and other healthcare professionals to coordinate patient care
• Monitor patient progress and adjust treatment plans as necessary
• Educate patients, families, and caregivers on therapy techniques and home exercises
• Maintain accurate and timely documentation in accordance with state, federal, and facility guidelines
• Ensure compliance with all regulatory and facility standards
• Contribute to team meetings and care planning for residents
