Job summary

Compassionate Care Home Health is seeking a dedicated Physical Therapist to provide patient care in Bakersfield and surrounding areas. Responsibilities include evaluating patients, developing and implementing therapy plans, and educating families on home-based rehabilitation.

Qualifications

• Master's or doctoral degree in Physical Therapy from an approved program.
• Licensed to practice as a Physical Therapist in California.
• 1 year of PT experience; community/home health preferred.
• Good verbal and written communication and organization skills.
• CPR certification is current.
• Valid driver’s license and reliable vehicle.

Responsibilities

• Provide physical therapy services per physician's plan of care.
• Assess and document patient progress and home environment.
• Instruct patients and caregivers in therapeutic techniques and devices.
• Coordinate discharge planning and referrals for additional services.
• Supervise PT assistants as per policy and state regulations.
• Instruct and evaluate home health aides as needed.
