Crest Haven Nursing and Rehab in New Jersey is seeking a compassionate Physical Therapist (PT) to join our team. You will conduct patient assessments and implement individualized treatment plans to promote mobility and independence.

You will work with an interdisciplinary team to provide high-quality care, document progress, and participate in ongoing education to stay current with best practices in physical therapy.

Responsibilities

• Conduct initial and ongoing assessments of patients to identify physical therapy needs and develop individualized treatment plans
• Provide physical therapy interventions, including but not limited to, exercises, stretches, and other manual therapies
• Collaborate with other healthcare professionals to develop and implement comprehensive treatment plans
• Communicate effectively with patients, families, and other healthcare professionals to ensure optimal care
• Maintain accurate and detailed records of patient treatments and progress
• Participate in in-service training and education to stay current with best practices and industry developments
