Rossvillehrc is seeking a Physical Therapist to develop individualized therapy plans for SNF patients, including assessment, planning, implementation, and education to maximize functional outcomes. You will collaborate with other health professionals for integrated care.

The position emphasizes direct patient care, documentation, and ongoing professional development. A valid PT license is required, with travel as needed, in a supportive team environment.

Qualifications

• Graduate degree in Physical Therapy from an accredited program.
• Current license to practice from the State Board of Physical Therapy Examiners.

Responsibilities

• Provides direct patient care and evaluates functional needs and outcomes.
• Designs and implements a care plan based on assessments.
• Treats patients within defined standards of care.
• Directs support staff to achieve patient goals.
• Communicates with professionals and staff to reach positive outcomes.
• Follows department policies, safety standards and infection control.
• Documents evaluations, goals and plans; maintains signatures.
• Pursues ongoing professional development and literature review.
