Responsibilities

• Screen and evaluate patients with physical functioning disorders and/or wound care, developing care plans.
• Deliver rehabilitative, skilled, and medically necessary treatment interventions.
• Administer therapies including manual techniques, ambulation, exercises, modalities, and wound care.
• Provide consultation to patients, families, and other providers.
• Generate comprehensive discharge summaries ensuring regulatory compliance.
