Duties span home and school settings, including companionship and basic health tasks under the care plan. The position requires state licensure or certification as required, 1 year of relevant experience preferred, and current BLS/PPD or Chest X-Ray as part of the minimum requirements.

Qualifications

• Strong communication with clients, families and team members.
• Proficiency in English; clear written and verbal skills.
• Able to use basic computer systems and electronic records.

Responsibilities

• Provide day-to-day living support and personal care.
• Assist with mobility, transfers and ambulation.
• Support health-related tasks and hygiene per plan of care.
• Assist in school settings with student daily activities and supervision.
