Job summary

Geisinger in Wilkes-Barre is seeking a Personal Care Assistant to support adults at the Day Health Center and in home settings. You will perform direct care, monitor nutrition, vital signs, mobility, and medication compliance, while ensuring safety and quality in every interaction.

The role requires a high school diploma or GED, BLS/CPR within 6 months, a valid driver's license, and strong communication skills to collaborate with an interdisciplinary team and families.

Qualifications

• High School Diploma or Equivalent (GED) required.
• Minimum of 1 year related work experience preferred.
• BLS (Basic Life Support) certification within 6 months.
• CPR certification within 6 months (AHA).
• Valid Driver's License required.
• Strong communication and interpersonal skills.

Responsibilities

• Provide personal care assistance to participants at the Adult Day Health Center and in participants' homes.
• Perform direct participant care including personal care, nutrition/hydration, vital signs, ambulation, transfers, ROM, med compliance, and incontinence care.
• Execute indirect care: light housekeeping, laundry, supplies, medication delivery as needed.
• May be required for on-call coverage for after-hours emergencies in the participant’s home.
• Maintain safety with proper body mechanics, gait belt, and back brace during transfers and ambulation.
• Collect data for mandatory requirements and participate in quality improvement projects.
• Complete accurate documentation for services per policy.
• Communicate effectively with participants and co-workers to foster self-help.
• Work with interdisciplinary team, attend team meetings, complete trainings.
