Job summary

Qualifications

• Must be at least 18 years old.
• Provide personal care services to client per ISP.
• Work under supervision and report changes promptly.

Responsibilities

• Provide personal care services to the client as outlined in the ISP.
• Perform personal care tasks in client’s residence or independent living environment.
• Report and communicate changes in a client’s condition to a supervisor immediately upon discovery.
• Report client service requests or changes within 24 hours of the next business day.
• Follow emergency medical training while attending the client.
• Report any emergencies to supervisor or provider as required by policy.
• Report suspected abuse, neglect, or exploitation of clients to a supervisor on discovery.
