The role emphasizes rounds on patients, post-discharge follow-ups, and collaboration with clinicians to address concerns in a timely manner. A high school diploma is required, with an associate degree preferred, and BLS certification recommended.

Qualifications

• Minimum education: High school diploma or equivalent.
• Associate’s degree in a healthcare related field preferred.
• AHA Basic Life Support (BLS) certification

Responsibilities

• May prepare reports, maintain filing systems, enter data and maintain databases.
• Communicate with patients, families, visitor, providers, and staff.
• Collaborate with stakeholders across the organization to work towards an appropriate resolution.
• Use a hands-on consultative approach that balances patient experience goals with unit operations.
• Provide professional and empathetic attention to patients and families, document, investigate, and provide feedback to leadership.
• Complete rounds on patients in Acute Care Units to communicate with patients, caregivers, and families.
• Complete post-discharge callbacks to follow-up with patients and families regarding care received during stay.
• Completes follow-up and notifies appropriate personnel regarding issues or concerns.
