Job summary

Advocate Health Care is seeking a compassionate patient care assistant to support daily activities and basic nursing tasks in a busy clinical setting. You will help with hygiene, mobility, nutrition, and documentation while adhering to safety and privacy standards.

As part of the patient care team, you will accompany patients, monitor conditions, and report changes promptly to Registered Nurses, ensuring a respectful, supportive environment for patients and families.

Qualifications

• High school diploma or GED equivalent.
• Current CPR.

Responsibilities

• Never leave a patient unattended and monitor continuously.
• Follow policies for precautions and emergency plans.
• Provide AM/PM care including bed making, bed bath, oral and denture care, shave, back rub and skin care.
• Assist with ambulation, positioning, and getting up to chair and back to bed.
• Assist with toileting needs and diapering; monitor skin breakdown risk.
• Provide diet-related tasks: tray setup, menu selection, intake/output notes.
• Respond to call lights and communicate requests to the RN.
• Maintain patient confidentiality and communicate respectfully with patients and families.
• Document patient information and communicate changes to RN; convey shift reports to care team.
