Qualifications

• CNA or MA certification preferred.
• 1-2 years in healthcare or home health/senior care preferred.
• Valid driver's license and reliable transportation required.

Responsibilities

• Coordinate patient appointments and provider schedules with senior living communities.
• Serve as the primary liaison between patients, families, providers and facility staff.
• Document patient health information accurately in the EMR and ensure HIPAA compliance.
• Assist in coordinating ongoing patient care and follow-up services.
