Nexus Children's Hospital - Shenandoah is seeking a dedicated Patient Care Assistant to join our 50-bed facility in Shenandoah, TX. This full-time role features 12-hour day and night shifts, and you will provide hands-on care as part of an interdisciplinary team.

You will assist with daily living activities, document vitals, and support a safe, compassionate environment. A high school diploma and 1 year of care experience are required, with BLS and de-escalation training expected after hire.

Formación

• High school diploma or equivalent required.
• At least one year of direct patient care in hospital or behavioral health settings.
• BLS certification from recognized providers required, six months remaining validity.
• CNA training or certification preferred.

Responsabilidades

• Assist patients with bathing, dressing, grooming, feeding, and mobility.
• Obtain and document vital signs; report changes promptly to nursing staff.
• Perform point-of-care testing and specimen collection as trained.
• Support one-to-one patient monitoring and 15-minute observation checks.
• Participate in bedside shift reports and share observations with the care team.
• Engage patients in therapeutic activities and group programming.
• Use de-escalation techniques to support safety for patients and staff.
• Assist licensed nurses with wound care and minor procedures as directed.
• Maintain documentation and adhere to infection prevention and privacy policies.
• Help orient and support new team members as needed.
