Job summary

Nexus Children’s Hospital – Shenandoah in Shenandoah, TX is seeking a Patient Care Assistant to provide hands-on care and support as part of an interdisciplinary team. You will help patients with daily activities, monitor vitals, document care, and report changes promptly to nursing staff in a safe, compassionate environment.

This full-time role offers 12-hour day or night shifts. Ideal candidates have a high school diploma, at least one year of direct patient care experience, and BLS

Qualifications

• High school diploma or equivalent required.
• At least one year of direct patient care experience in a hospital, behavioral health, rehabilitation, or long-term care setting required.
• Experience supporting patients with behavioral health conditions, cognitive impairments, or psychiatric disorders.
• Strong communication skills and the ability to provide compassionate care while maintaining professional boundaries.
• Ability to use an electronic health record and document care accurately.
• BLS certification from the American Heart Association or American Red Cross required, valid for at least six months from date of hire.
• Ability to complete required de-escalation training within 30 days of hire.

Responsibilities

• Assist patients with bathing, dressing, grooming, feeding, turning, positioning, mobility, and other activities of daily living.
• Obtain and document vital signs; promptly report changes in a patient’s condition.
• Perform point-of-care testing and specimen collection within training and procedures.
• Complete 15-minute observation checks and support one-to-one monitoring.
• Participate in bedside shift reports and share observations with the care team.
• Engage patients in therapeutic activities and group programming.
• Use de-escalation techniques to support safety.
• Assist licensed nursing staff with wound care and minor procedures.
• Maintain accurate documentation and follow infection control and privacy procedures.
• Help orient and support newer team members.
