Qualifications

• Education/Experience Required: - High school graduate or equivalent. - Ability to read, write and follow instructions as required. Knowledge, Skills & Abilities Required:
• Good communication skills
• Courteous and professional interpersonal skills
• Basic math calculation skills
• CPR (Heart Saver) certified within 6 months of employment
• Certification as a Nursing Assistant by the State of Illinois

Responsibilities

• Introduces self and role to patient at start of shift.
• Provides water and assists with patient needs as directed.
• Responds to call lights and patient requests.
• Assists with patient mobility and daily care as directed by RN.
• Documents patient information on appropriate forms.
• Communicates patient/family concerns to nursing staff.
• Participates in admission, transfer and discharge processes as assigned by RN.
