Qualifications

• Requires HS diploma or GED.
• Six months of healthcare experience; at least one year in-home patient care preferred.
• LP, CNA, Caregiver, PCA, Paraprofessional, or HHA experience preferred but not required.
• Ability to react professionally in emergencies per protocol.

Responsibilities

• Assist residents with care plan management, service scheduling, and medications.
• Provide direct and indirect patient care including personal care, meals, hygiene, and mobility in a group home setting.
• Complete patient-specific paraprofessional documentation in medical records.
• Report potential compliance issues per Employee Handbook.
• Model ethical behavior, professional standards, and integrity.
