AccentCare, Inc. in Los Angeles, CA, is seeking a full-time LVN/LPN in Palliative Care who is compassionate and dedicated to delivering quality end-of-life care.

This on-site role covers Southeast LA County including Downey and Montebello, with a schedule Monday to Friday. You will work under an NP or physician to execute care plans, coordinate with interdisciplinary teams, and support patients and families.

Qualificações

• Graduated from an approved school of professional nursing.
• One year experience as a licensed practical nurse / Palliative experience preferred, required in California.

Responsabilidades

• Under the direction of an NP or physician, assist in execution of the care plan, including routine and emergent visits
• Contributes to the on-going care planning process, especially regarding medical needs, with the care team and other agencies as required
• Initiates and maintains communication with the NP, interdisciplinary team, Palliative Director, and contracted medical groups to coordinate optimal care and use of resources for the patient/family
• Proves support for the Palliative patient and family as new clinical needs are identified or questions arise, with the referral to the clinician as appropriate
• Gathers information regarding the patient for appropriateness for other service lines that will best meet patient needs such as Hospice
