Qualifications

• MSW required; NYS license to practice, LCSW preferred.
• Experience in palliative/hospice care and behavioral health preferred.
• Knowledge of CQI processes, Utilization Review, and Case Management.
• Excellent verbal and written communication skills; NYS driver's license required.

Responsibilities

• Participation in Plan of Care and provision of social work services.
• Determine admission status and level of care with palliative care team.
• Coordinate discharge planning and communication with payors and families.
• Document interventions and maintain timely care plan notes.
• Engage in professional development and CQI initiatives.

Education
