Job summary

Hertford Rehabilitation and Healthcare Center in Chesapeake, Virginia is seeking an Occupational Therapist to provide high-quality rehab services to residents, focusing on independence and well-being. You will evaluate, treat, and document progress, collaborating with the care team in a fast-paced skilled nursing facility.

Candidates should have a degree in Occupational Therapy and a current license; SNF experience preferred.

Qualifications

• Graduate of an accredited university with a B.S., MA, M.S. or Doctorate in Occupational Therapy.
• SNF experience preferred.
• Current license and/or registration in the state.
• Strong communication and interpersonal skills.
• Ability to work in a fast-paced environment.
• Compassion, empathy, and a strong patient-centered approach.

Responsibilities

• Provide comprehensive occupational therapy evaluations, assessments, and treatment plans for residents in accordance with medical orders and facility policies
• Document patient progress and data accurately and in a timely manner
• Provide individualized therapy sessions to residents, utilizing a variety of modalities and techniques
• Collaborate with the interdisciplinary team to develop and implement effective care plans
• Participate in staff meetings, education sessions, and in-service training as needed
• Maintain accurate and up-to-date records of patient care and therapy sessions
• Adhere to facility policies and procedures, including those related to patient confidentiality and Federal Resident Rights
• Demonstrate a commitment to excellence in patient care and professionalism in all interactions
