Job summary

Hertford Rehabilitation and Healthcare Center in Chesapeake, Virginia is seeking a dedicated Occupational Therapist to join our team. You will provide comprehensive OT evaluations, plan treatments, and collaborate with the interdisciplinary team to help residents achieve rehabilitation and independence.

The ideal candidate holds an accredited OT degree, state license, strong communication, and a patient-centered approach.

Qualifications

• Graduate of an accredited university with a B.S., MA, or M.S. or Doctorate in Occupational Therapy.
• SNF experience preferred
• Current license and/or registration in the state (as applicable)
• Strong communication and interpersonal skills
• Ability to work effectively in a fast-paced environment
• Compassion, empathy, and a strong patient-centered approach

Responsibilities

• Provide comprehensive occupational therapy evaluations, assessments, and treatment plans for residents in accordance with medical orders and facility policies
• Document patient progress and data accurately and in a timely manner
• Provide individualized therapy sessions to residents, utilizing a variety of modalities and techniques
• Collaborate with the interdisciplinary team to develop and implement effective care plans
• Participate in staff meetings, education sessions, and in-service training as needed
• Maintain accurate and up-to-date records of patient care and therapy sessions
• Adhere to facility policies and procedures, including those related to patient confidentiality and Federal Resident Rights
• Demonstrate a commitment to excellence in patient care and professionalism in all interactions
