Job summary

Brooks Rehabilitation in Jacksonville, FL seeks a qualified Occupational Therapy Assistant for PRN coverage at the Bartram Crossing campus. The role provides patient-centered therapy under the direction of a licensed OT, delivering individualized treatment plans.

You will participate in assessments, discharge planning, documentation, and collaboration with the rehab team to promote functional independence for patients.

Qualifications

• Associate of Science Degree in Occupational Therapy.
• Licensed as an Occupational Therapy Assistant in the State of Florida.
• Hands-on BLS Card.
• Maintains confidentiality of all patient information.
• Adheres to policies and procedures specific to patient rights.
• Maintains a clean and safe environment; identifies and reports hazards.

Responsibilities

• Assessment and Plan of Care/Treatment.
• Evaluates patient's occupational performance at discharge according to department policy and procedures.
• Collaborates with the supervising Occupational Therapist on a treatment program based on patient/family needs, goals, strengths, age, abilities, and preferences.
• Performs patient treatment through the use of appropriate occupational therapy interventions.
• Re-evaluates patient's status to modify/terminate the treatment program as appropriate.
• Maintains accurate daily billing charges and productivity log.
• Documents evaluations, progress notes, discharge recommendations, and all other forms in accordance with department policy.
