Job summary

InnovAge is seeking a Certified Occupational Therapy Assistant (COTA) to administer occupational therapy under physician orders and the plan of care. You will work under the direction of the Occupational Therapist to develop goals and adapt activities for participants, while documenting progress and educating families in daily living activities.

Responsibilities include collaboration with the OT, maintaining records, and supporting continuous quality improvement within the program.

Qualifications

• Current NBOT certification required.
• Current CPR certification or ability to obtain.
• Requires personal transportation, valid driver’s license and insurance.
• 1+ year working with the frail or elderly.

Responsibilities

• Provide physician prescribed occupational therapy under a plan of care.
• Develop treatment goals with supervision from the OT.
• Adapt environment, tools, and activities to participant needs.
• Document progress notes and care plans in a timely manner.
• Educate participant and family on daily living activities.
• Participate in care planning with health care team.
• Report changes in patient condition to the OT.
• Maintain treatment areas and inventory.
