Ideal candidates hold a nursing diploma and RN license with BLS; BSN preferred. You will collaborate with care partners and healthcare teams to develop individualized care plans, ensure cultural sensitivity, and uphold ongoing learning in a dynamic homecare environment.

Qualifications

• Must hold a nursing diploma with RN license and BLS certification.
• BSN preferred and prior acute care experience.
• Ability to lift up to 50 lbs and perform transfers/CPR.
• Develops individualized care plans with patients and families respecting cultural values.

Responsibilities

• Develops individualized care plans in collaboration with patients and care partners.
• Engages in effective communication, education, and shared decision-making with patients, families, and healthcare teams.
• Uses evidence-based practices, health IT, and quality improvement to improve safety and outcomes.
• Participates in team efforts, committees, and professional development.
• Demonstrates respect, inclusion, and a commitment to learning.
• Physical demands include lifting up to 50 lbs and transfers/CPR.
