Qualifications

Responsibilities

• Completes bio-psycho-social and environmental assessments for patients.
• Provides bio-psycho-social interventions including counseling and referrals as needed.
• Consults with physicians and staff on patient and family psychosocial issues.
• Coordinates care and resources across the health care team.
• Documents assessments, plans, and data in the EMR.
• Coordinates care with mental health and substance abuse facilities.
• Provides weekend/holiday social work coverage.
• Adheres to NASW/ACA ethics and licensure rules.
