Job summary

Saint Alphonsus Health System is seeking an Oncology Social Worker to provide advocacy, education, and timely bio-psycho-social interventions for patients and families. The role emphasizes collaboration with medical staff and community resources to support care transitions and optimize outcomes.

Requirements include a Master’s degree in social work or counseling and licensure as LMSW or LPC in Idaho, with LCSW/LCPC preferred.

Qualifications

• Licensed Master Social Worker (LMSW) or Licensed Professional Counselor (LPC) in Idaho.
• LCSW or LCPC preferred.
• Master’s degree in social work or counseling required.
• Basic Life Support certification from the American Heart Association or American Red Cross is required.

Responsibilities

• Knows, understands, incorporates, and demonstrates the Organizational Mission, Core Values, and Vision in behaviors, practices, policies and decisions.
• Completes bio-psycho-social and environmental assessments on identified and/or referred patients.
• Provides bio-psycho-social and environmental interventions for patients and families/caregivers, including brief counseling and referrals as needed.
• Provides consultations to physicians and hospital staff regarding patient and family bio-psycho-social issues.
• Collaborates with health care team members to meet emotional, mental, environmental, bio-psycho-social and spiritual needs.
• Participates in care coordination activities for patients needing social work and complex case management.
• Documents assessments and interventions in the EMR; maintains primary and backup plans.
• Documents data elements in the electronic database; coordinates care with mental health and/or substance abuse facilities.
• Weekend and holiday social work coverage; participates in department meetings and sub-teams.

Master’s degree in social work or counseling
