Thomas Jefferson University Hospitals, Inc. in New Jersey is seeking an Oncology Nurse Navigator to coordinate care for cancer patients and their families, ensuring timely diagnostic work-up, results reporting and communication among the multidisciplinary team.

This role advocates for patients, supports transitions of care, educates about disease and treatment options, and participates in tumor boards. BSN is required; 3 years healthcare experience with oncology preferred.

Qualificações

• Graduate of an accredited school of nursing.
• BSN or completion of BSN within 3 years of hire preferred.
• Three years of healthcare/medical experience; oncology/cancer care preferred.

Responsabilidades

• Provides expert individualized care coordination to patients and families.
• Advocates for patients and promotes timely, competent, compassionate care.
• Educates patients and families about disease process and potential side effects.
• Assists patient with treatment decisions and uses decision aids as appropriate.
• Attends and supports Tumor Boards.
• Supports transitions of care and coordination between sites.
• Provides clinical consultation to the Patient Navigators.
