Job summary

Saint Joseph Health System is seeking an Oncology Patient Navigator to assist physicians and Disease Site Management Teams in coordinating cancer care from diagnosis through treatment and recovery, presenting intervention options and linking patients to center and community resources.

The ideal candidate has a Bachelor of Nursing; MS in Nursing preferred; Indiana RN license preferred; 3+ years acute clinical experience (oncology preferred); Spanish helpful.

Qualifications

• Bachelor of Nursing Degree preferred.
• MS in Nursing preferred.
• RN Indiana license preferred.
• Three or more years of acute clinical experience.
• Oncology experience preferred.
• Discharge planning or Case management acceptable.
• Ability to speak Spanish preferred.

Responsibilities

• Coordinate care with physicians and Disease Site Management Teams from diagnosis through treatment and recovery.
• Develop and coordinate oncology educational materials for patients and families as defined by Disease Site Teams and physicians.
• Provide patient and staff education, group programs, resources maintenance and data collection to facilitate high‑quality, patient‑centered care.
