Job summary

The Keystone Distinction Group seeks a compassionate Hospice Nurse (LPN or RN) for 7on/7off coverage in the East Kansas City Metro area. The role requires after-hours on-call duties Monday through Thursday evenings and Friday through Monday mornings, with field and telehealth responsibilities to support patients at home or in facilities.

Responsibilities include symptom management, care coordination with an interdisciplinary team, and family education to ensure high-quality end-of-life care

Qualifications

• Active MO/KS Nursing license.
• CPR certification current.
• 2–5 years hospice/palliative care experience.
• Knowledge of hospice services for patients and families.
• Strong organizational skills.
• Effective verbal and written communication.
• Analytical problem-solving abilities.
• High attention to detail.

Responsibilities

• Provide after-hours hospice nursing care (evenings and weekends).
• Assess and respond to patient/caregiver needs via telephone contact, home visits, or facility visits.
• Coordinate care with an interdisciplinary team.
• Educate patients and families on symptom management and care planning.
• Support and monitor patients in home and facility settings within service area.
