Qualifications

• Certification: BLS within 30 days - American Heart Association (AHA).
• Licensed Occupational Therapist (OT) - IDFPR.
• Education: Bachelor's Degree in Occupational Therapy.

Responsibilities

• Demonstrates ability to perform patient transfers with assistance from bed, chair and floor.
• Maintains safe body mechanics to reduce risk of injury to self, staff and patients.
• Designs an individualized plan of care aligning goals, treatment, outcomes, and discharge plans.
• Adjusts care based on patient response and evidence.
• Educates patients, families, caregivers, students, and other health care providers using effective teaching methods.
• Evaluates clinical findings to determine physical therapy diagnoses and outcomes of care.
• Performs examinations using reliable methods relevant to chief complaint and history.
• Screens patients to determine need for further therapy services.
• Supervises COTA and rehabilitation technicians; provides feedback.
• Documents evaluations and plans of care per department policy.
