Qualifications

• Certified nursing assistant certification required in California.
• Ability to document vital signs and patient observations per policy.
• Assist with patient hygiene, mobility and transfers as directed.
• Willingness to report issues to the RN and follow unit procedures.

Responsibilities

• Obtain vital signs (temperature, pulse, respirations, BP and pain level) and document.
• Assist with hygiene, positioning, ambulation and daily care per care plan.
• Record intake/output and weights; communicate changes to RN.
• Maintain safe patient environment and observe isolation procedures.
• Assist with equipment storage, supply restocking and charting as directed.
• Demonstrate effective communication and time management with the team.
